When performing liposuction, what we call the subcutaneous fat tissue also has two layers: namely, there is a superficial fat layer and a deep fat layer. But surgically speaking, as a plastic surgeon performing liposuction, how do we approach this? We divide these layers under the skin into three: a superficial, a middle, and the deepest layer. These layers, of course, can show variations across different regions of the body. For example, on the back, while the superficial and middle layers are much tighter and firmer in structure — meaning it is harder to perform liposuction there — the deep layer of the back is softer. But in a thigh or the abdomen, these can differ. While performing liposuction, one must pay attention to all this and be aware of them. We must avoid entering the superficial layer. In the superficial layer, there is a vascular network that supplies the skin, and when we enter the superficial layer we can damage that vascular network. We can disrupt the vascularity and blood supply of the skin. This can lead to what we call contour irregularities in the skin, causing a dimpled, indented and bumpy appearance, as well as causing necrosis by disrupting the skin's circulation — meaning non-healing wounds and similar issues on the skin. That's why it is truly necessary to be careful about this. Consequently, in liposuction, without damaging the superficial layer but by properly evacuating the underlying fat layers, we can reveal the definition of the muscles and other underlying structures — meaning the body's own natural contours — in a much better way.The Dramatic Silhouette

The secret to a dramatic silhouette isn't always about what you add — it's about how strategically you remove. With liposuction, how do we do this? In the waist area, compared to other anatomical regions, there is a different situation: under the skin of the waist, the fat layer is usually larger and thicker. The Liposuction Process
- Consultation: target areas, fat amount and skin elasticity are assessed.
- Method and anaesthesia: classic/VASER/laser and general or local+sedation are planned by area.
- Procedure: fat is removed with a cannula through tiny entries; fat transfer may be done in the same session.
- Compression garment: given to reduce swelling and help the skin settle.
- Same-day/observation: most return to daily life quickly; check-ups are planned.
- Stay: a short stay in Istanbul may be advised by case.
Recovery, Result and Candidacy
- A compression garment is usually worn for a few weeks; swelling subsides over time.
- Most return to desk work within a few days; to heavy sport usually after a few weeks.
- The result becomes clear as swelling settles and usually settles over a few months.
- Permanence: removed fat cells do not return; but weight gain can enlarge remaining cells, so a stable weight matters.
- Candidacy: localised excess fat, good skin elasticity and a stable weight are expected.
